Collegiate Initiated Member Form
Your chapter's FEO should complete this form immediately after Initiation of Probationary Members. Hitting “submit” once completed will transmit it directly to the VP Collegiates.
Remember to have each member complete the Collegiate Member Profile and send these into the National VP Collegiates. You should also complete the Membership Certificate Form. All Initiation Fees should be sent to the National Treasurer.
